Optional services can be elected when opening a Master Account or any time in the future

Direct Deposit of State Aid

If you would like the State of Missouri or your county to send your state aid / tax payments directly to your MOSIP account, complete the State of Missouri Vendor ACH/EFT Application. We have inserted our bank information on the enclosed form for your convenience.
 

Additional Accounts

To open additional, separate account(s) under the same entity name and TIN, complete an Additional Account Authorization Form and Permissions Form for each sub-account. 

Bond Issue Information Form

The information provided on the Bond Issue Information Form will be used by the Investment Advisor to prepare reports as to the expenditure of bond proceeds for the Investor as more fully described under “rebate exception services”.
